Welcome to Safe Transitions! We help people with mental illness and disabilities by providing quality, specialized services. Currently, services are provided in Minnesota in 11 adult foster care settings and for many adults and children in their own homes.

The mission of Visionary Home Care is to help people live their lives to the fullest by providing dependable, compassionate, Visionary Home Care assistance.
The goals of Visionary Homes are to:
- Promote active, fulfilling lifestyles for seniors and people with disabilities or illnesses
- Help busy people or new moms and dads with home care assistance
- Provide dependable, compassionate, cost effective, creative non-medical home care assistance
- Decrease isolation and loneliness
- Lessen the burden and guilt of loved ones and caregivers
- Provide peace of mind and respite for loved ones and caregivers
- Maintain the ability for seniors and those with disabilities or illnesses to stay in the comfort and privacy of their homes
- Help people stay independent to the best of their ability
- Encourage people and loved ones to take control of their own home care needs
- Listen and adapt to the needs of our clients
- Recognize and assist with issues surrounding changes in mental health
- Ensure safety at home for seniors and those with disabilities or illnesses
- Respect the privacy of clients, their homes, and their families
- Brighten up someone's day by providing gift certificates for your friends, loved ones, or someone in need of a little help
- Provide employment opportunities for people who love caregiving
- Provide caregivers with training and education based on the individual needs of their clients.
